I have had a mermaid NPC admit she was a mermaid after my sim became best friends with her (full friendship bar). You need to have a really high level relationship for them to admit they are mermaids.

Do you have to be in water to be a mermaid?
You’ll need to be in water for them to transform into a mermaid. When they’re on land, they’ll look and walk like any other regular Sim. The other things you need to keep in mind is that any mermaid sims will have their Hygiene need replaced by Hydration.
